ABSTRACT

Background and objectives: children's food-related lifestyle brought changes in their weight status and eating patterns. We aimed to investigate the association between food consumption pattern and excess weight in preschoolers aged six. Methods: a cross-sectional analysis of a prospective birth cohort, in 2004, in Feira de Santana-BA. Food consumption frequency was verified, characterized by patterns and established through a food frequency questionnaire, validated in a previous study, in pattern 1: milk, vegetables, cereals, legumes, fruits and fish; pattern 2: snacks, soda/artificial juice, oils, sweets and coffee/tea; pattern 3: sausages, fast food, ketchup/mayonnaise and egg; pattern 4: red meat and chicken. Excess weight was defined according to the World Health Organization criteria. The main association was assessed using Pearson's chi-square test and Poisson regression. Results: a total of 618 children were investigated. Excess weight occurred in 28.6%, and pattern 3 frequency was 68%. In the bivariate analysis, only pattern 3 was associated with children's excess weight (PR: 1.23; 95%CI: 1.01-1.63). In the multivariate analysis, the prevalence of excess weight children who consumed pattern 3 was 50% (adjustedPR: 1.50; 95%CI: 1.01­1.93). Conclusion: moderate/high consumption of sausages, fast food, ketchup/mayonnaise and eggs was associated with excess weight among children in the sample.(AU)


Justificativa e objetivos: o estilo de vida das crianças relacionado à alimentação trouxe mudanças no estado de peso e no padrão alimentar. Objetivou-se averiguar a associação entre o padrão de consumo alimentar e o excesso de peso em pré-escolares aos seis anos de idade. Métodos: análise transversal, de uma coorte prospectiva de nascimento, em 2004, em Feira de Santana-BA. Verificou-se a frequência do consumo alimentar, caracterizado por padrões e estabelecido por meio do questionário de frequência alimentar, validado em um estudo prévio, em padrão 1: leite, verduras, cereais, leguminosas, frutas e pescados; padrão 2: salgadinhos, refrigerantes/sucos artificiais, óleos, doces e cafés/chás; padrão 3: embutidos, fast food, ketchup/maionese e ovo; padrão 4: carnes vermelhas e frango. O excesso de peso foi definido segundo os critérios da Organização Mundial da Saúde. A associação principal foi avaliada mediante Teste Qui-Quadrado de Pearson e regressão de Poisson. Resultados: investigaram-se 618 crianças. O excesso de peso ocorreu em 28,6%, e a frequência do padrão 3 foi de 68%. Na análise bivariada, somente o padrão 3 se associou com o excesso de peso da criança (RP: 1,23; IC95%: 1,01-1,63). Na análise multivariada, a prevalência de crianças que consumiam o padrão 3 com excesso de peso foi de 50% (RPajustado: 1,50; IC95%: 1,01­1,93). Conclusão: o consumo moderado/alto de embutidos, fast food, ketchup/maionese e ovo se associou com o excesso de peso entre as crianças.(AU)


Justificación y objetivos: el estilo de vida de los niños relacionado con la alimentación trajo cambios en su estado de peso y patrones de alimentación. El objetivo fue investigar la asociación entre el patrón de consumo de alimentos y el sobrepeso en preescolares de seis años. Métodos: análisis transversal de una cohorte prospectiva de nacimiento, en 2004, en Feira de Santana-BA. Se verificó la frecuencia de consumo de alimentos, caracterizada por patrones y establecida a través del cuestionario de frecuencia de alimentos, validado en un estudio previo, en el patrón 1: leche, verduras, cereales, legumbres, frutas y pescado; patrón 2: bocadillos, refrescos/jugos artificiales, aceites, dulces y café/té; patrón 3: salchichas, fast food, salsa de tomate/mayonesa y huevo; patrón 4: carnes rojas y pollo. El exceso de peso se definió según los criterios de la Organización Mundial de la Salud, la asociación principal se evaluó mediante la prueba de chi-cuadrado de Pearson y regresión de Poisson. Resultados: 618 niños fueron investigados. El exceso de peso se presentó en el 28,6% y la frecuencia del patrón 3 fue del 68%. En el análisis bivariado, solo el patrón 3 se asoció con el exceso de peso del niño (RP: 1,23; IC95%: 1,01-1,63). En el análisis multivariado, la prevalencia de niños con sobrepeso que consumieron el patrón 3 fue del 50% (RPajustado: 1,50; IC95%: 1,01­1,93). Conclusión: el consumo moderado/alto de salchichas, fast food, ketchup/mayonesa y huevos se asoció con sobrepeso entre los niños de la muestra.(AU)

ABSTRACT

Physical exercise is effective in modulating circulating inflammatory markers of obesity. However, little is known about the effects of interventions with physical exercise programs accompanied by nutritional and/or psychological guidance, configuring themselves as multicomponent programs. Thus, the aim of this review is to systematically evaluate the evidence related to the effects of mul-ticomponent weight-loss interventions in modulating circulating inflammatory markers in children and adolescents. Therefore, the following databases will be searched to identify all relevant articles: PubMed, SciELO, Lilacs, Web of Science, EMBASE, Scopus, SPORT Discus. Randomized con-trolled trials and quasi-experimental studies of children and adolescents (6 to 18 years old) will be included. Eligible interventions will target weight-related behaviors (including diet, physical activity, behavior modification and/or combinations thereof ). Two independent reviewers will select studies using Rayyan QCRI software and extract the data to a standard form. The main outcomes of the review will be the circulating values of leptin, interleukin 6 (IL-6) and tumor necrosis factor-alpha (TNF- α) in a quantitative way. To assess the methodological quality (or risk of bias) of individual studies, Effective Public Health Practice Project assessment tool will be used. The meta-analysis will be performed using the Review Manager software


O exercício físico é eficaz na modulação dos marcadores inflamatórios circulantes da obesidade. Porém, pou-co se sabe sobre os efeitos de intervenções com programas de exercícios físicos acompanhados de orientações nutricionais e/ou psicológicas, configurando-se como programas multicomponentes. Assim, o objetivo des-te protocolo é propor uma revisão sistematicamente das evidências relacionadas aos efeitos de intervenções multicomponentes para perda de peso na modulação de marcadores inflamatórios circulantes em crianças e adolescentes. Para tanto, as seguintes bases de dados serão pesquisadas para identificar todos os artigos relevantes: PubMed, SciELO, Lilacs, Web of Science, EMBASE, Scopus, SPORT Discus. Serão incluídos ensaios clínicos randomizados e estudos quase experimentais com crianças e adolescentes (6 a 18 anos). As intervenções elegíveis terão como foco a melhora dos comportamentos relacionados ao peso (incluindo dieta, atividade física, modificação de comportamento e/ou combinações dos mesmos). Dois revisores independentes selecionarão estudos usando o software Rayyan QCRI e extrairão os dados em um formulário padrão. O principal resultado da revisão serão os valores circulantes de leptina, interleucina 6 (IL-6) e fator de necrose tumoral alfa (TNF-α) de forma quantitativa. Para avaliar a qualidade metodológica (ou risco de viés) de estudos individuais será utilizada a ferramenta de avaliação Effective Public Health Practice Project. A meta-análise será realizada no software Review Manager

ABSTRACT BACKGROUND: Individuals who are overweight or obese often underestimate their size, and they are less likely to consider their weight status to be a health problem and consequently to make lifestyle changes. OBJECTIVES: To estimate the proportion of satisfaction with weight among adolescents classified as overweight/obese, according to sociodemographic factors, morbidities and health-related behaviors. DESIGN AND SETTING: Cross-sectional population-based study conducted among adolescents aged 10 to 19 years in the city of Campinas (SP), Brazil. METHODS: The sample (n = 217) included participants with self-reported weight and height who were classified as overweight or obese, based on body mass index (BMI) according to age-specific cutoff points recommended by the World Health Organization. Participants whose answer to the question: "Would you like to gain or lose weight?" was "no" (i.e. no change) were deemed to be satisfied with their body weight. Odds ratios and 95% confidence intervals (95% CI) were calculated using logistic regression. RESULTS: The proportions of the respondents who were satisfied with their weight were 75.8% (95% CI: 65.3-83.9) among the overweight adolescents and 24.2% (95% CI: 16.1-34.7) among the obese adolescents (P < 0.01). Satisfaction was lower among individuals aged 15 to 19 years (versus 10 to 14 years), those born outside of Campinas (versus in Campinas), those with ≥ 8 household appliances (versus < 8), and those reporting ≥ two health complaints (versus none). CONCLUSIONS: More than half of the overweight adolescents and almost a quarter of the obese adolescents were satisfied with their weight. These results support the need for strategies for healthy weight management among Brazilian adolescents.

Background: Obesity an important health problem which has become a worldwide epidemic that effects both developed countries and developing countries. Bariatric surgery is the most efficient cure for morbid obesity. Authors retrospectively reviewed the results of different techniques in bariatric surgery to assess the efficiency of initial operations.Methods: The medical files and operation notes and the one-year follow-up of patients were reviewed. The outcomes of sleeve gastrectomy (SG) and gastric roux-n-y bypass (RYGBP) operations were reviewed and their effects on excess weight loss were investigated.Results: Overall, there were 97 patients included in the study, the majority of which were female (87/97, or 90%). The mean age of the SG group was 42.01±8.48 years versus 36±8.32 years in the RYGBP group, with p=0.007. Excess weight loss percentages were 51.7% and 57% for sleeve gastrectomy and RYGBP, respectively, p=0.491. Both operations are effective in reducing comorbidities.Conclusions: In the RYGBP procedure there is more excess weight loss than the results of SG procedure but the difference is not statistically significant. In both procedures, the initial results on weight loss and reduction of comorbidities are comparable. Because SG is less traumatic and relatively easier to perform, it can be the preferred operation for morbid obesity.

OBJECTIVE: Evaluate the association between early menarche and anthropometric/body composition variables in young female university students. METHODS: Cross-sectional study involving female university students at a public university in northeastern Brazil. Anthropometric and body composition data were collected, along with a self-report of age at menarche. The conceptual model considered demographic, socioeconomic and lifestyle variables as well as the perception of weight in childhood and at menarche. RESULTS: Among the 162 students analyzed, 62.3% were less than 20 years of age. The frequency of underweight and overweight was 8.6% and 22.3%, respectively. The frequency of above-average body fat and obesity was 32.1% and 14.8%, respectively. Early menarche was reported in 35.2%. The frequency of self-reported excess weight in childhood and at menarche was 31.3% and 25.9%, respectively. A statistically significant association was found between early menarche and height < 1st tertile (< 1.62 m). A tendency toward an association was found between early menarche and both weight, circumference in the risk range and the perception of excess weight in childhood and at menarche. CONCLUSION: Current excess weight was more associated with excess weight in childhood and at menarche than the occurrence of early menarche.


OBJETIVO: Evaluar asociación entre menarquia precoz, parámetros antropométricos y de composición corporal en jóvenes universitarias. MÉTODOS: Estudio transversal en 162 jóvenes universitarias del Nordeste brasileño. Se recogieron datos antropométricos, de composición corporal y relato sobre la edad de la menarquia. Se consideraron, además, variables demográficas, socioeconómicas, estilo de vida e histórico del peso. RESULTADOS: De las 162 jóvenes el 62,3% tenía menos de 20 años de edad. Un 8,6% y 22,3% presentaron bajo peso, y exceso de peso respectivamente. La frecuencia de grasa corporal y obesidad por encima de la media fue de 32,1% y 14,8%, respectivamente. La menarquia temprana fue reportada en el 35,2% de los casos. El 31,3% y 25,9% repostaron tener exceso de peso en la infancia y en la menarquia respectivamente. Se verificó asociación estadísticamente significativa entre menarquia precoz y talla <1 tercil (<1,62cm). Además de una tendencia de asociación entre menarquia precoz, circunferencia de la cintura y exceso de peso, tanto en la infancia como en la edad de menarquia. CONCLUSIÓN: el exceso de peso actual está, mas asociado con el exceso de peso en la infancia y la edad de la menarquia que con la menarquia precoz.

Introdução: O estudo visa investigar a taxa de prevalência e a tendência temporal do excesso de peso em gestantes usuárias da Atenção Primária à Saúde de Macaé, entre 2010 e 2018. Método: Realizou-se estudo quantitativo, de série temporal, com a base de dados do Sistema de Vigilância Alimentar e Nutricional (Sisvan WEB), de acesso público, referente ao diagnóstico nutricional de gestantes ≥20 anos usuárias da Atenção Primária à Saúde do município de Macaé, no período de 2010 a 2018. Analisaram-se 4.279 registros de gestantes no período de nove anos. As variáveis estudadas foram: ano de coleta e registro dos dados; baixo peso; peso adequado; sobrepeso; obesidade; excesso de peso (sobrepeso e obesidade). A análise de tendência do excesso de peso foi realizada por meio de modelos de regressão linear simples, considerando-se o nível de significância de 5%. Resultados: As prevalências do excesso de peso em gestantes foram elevadas e ascendentes: 2010 (40,2%), 2011 (37,5%), 2012 (46,8%), 2013 (48,8%), 2014 (50,9%), 2015 (51,8%), 2016 (54,4%), 2017 (57,6%) e 2018 (55,8%). Em todas as análises, detectou-se a tendência significativa e crescente, podendo-se destacar o desfecho do excesso de peso, que apresentou velocidade de incremento anual (ß1=2,35; p-valor<0,001) superior aos demais desfechos analisados em separado. Conclusão: Conclui-se que é elevada e crescente a prevalência do excesso de peso em gestantes usuárias da Atenção Primária à Saúde do município de Macaé no período estudado. (AU)


Introduction The study aims to investigate the prevalence rate and the time trend of excess weight in pregnant women receiving Primary Health Care in Macaé, between 2010 and 2018. Method: A quantitative, time series study was conducted with the open access database of the Food and Nutritional Surveillance System (Sisvan WEB), relative to the nutritional diagnosis of pregnant women> 20 years old, receiving Primary Health Care in the city of Macaé, between 2010 and 2018. Analyses were made of 4,279 records of pregnant women over a nine-year period. The study variables were: year of data collection and registration; low weight; adequate weight; overweight; obesity; excess weight (overweight and obesity). Trend analysis of excess weight was performed using simple linear regression models, considering the significance level of 5%. Results: Prevalence of overweight in pregnant women was high and rising: 2010 (40.2%), 2011 (37.5%), 2012 (46.8%), 2013 (48.8%), 2014 (50.9%), 2015 (51.8%), 2016 (54.4%), 2017 (57.6%) and 2018 (55.8%). All analyses showed a significant and growing trend, especially for the excess weight outcome, whose annual increase speed (£ 1 = 2.35; p-value <0.001) was higher than the other outcomes, when analyzed separately. Conclusion: Prevalence of excess weight in pregnant women receiving Primary Health Care in the city of Macaé was high and growing during the study period. (AU)

Abstract Objective: To describe the main predictors for excess birth weight in Brazilian children. Data sources: Systematic review carried out in the bibliographic databases: PubMed/MEDLINE, Cochrane, Scopus, Web of Science, and LILACS. The research in the gray literature was performed using the Google Scholar database. The bias risk analysis was adapted from the Downs and Black scale, used to evaluate the methodology of the included studies. Data synthesis: Using the classifications of fetal macrosomia (>4.000 g or ≥4.000 g) and large for gestational age (above the 90th percentile), 64 risk factors for excess birth weight were found in 33 scientific articles in the five regions of the country. Of the 64 risk factors, 31 were significantly associated with excess birth weight, with excess gestational weight gain, pre-gestational body mass index ≥25 kg/m2, and gestational diabetes mellitus being the most prevalent. Conclusion: The main predictors for excess birth weight in Brazil are modifiable risk factors. The implementation of adequate nutritional status in the gestational period and even after childbirth appears to be due to the quality and frequency of the follow-up of the mothers and their children by public health agencies.


Resumo Objetivo: Descrever os principais preditores para o excesso de peso ao nascer em crianças brasileiras. Fontes dos dados: Revisão sistemática feita nos bancos de dados bibliográficos: PubMed/Medline, Cochrane, Scopus, Web of Science e Lilacs. A pesquisa na literatura cinzenta foi feita na base de dados Google Acadêmico. A análise do risco de viés foi adaptada da escala de Downs e Black, usada para avaliar a metodologia dos estudos incluídos. Síntese dos dados: Com o uso das classificações macrossomia fetal (> 4.000 g ou ≥ 4.000 g) e grande para idade gestacional acima do percentil 90, foram encontrados 64 fatores de risco para excesso de peso ao nascer em 33 artigos científicos nas cinco regiões do país. Dos 64 fatores de risco, 31 foram significativamente associados a excesso de peso ao nascer, os mais prevalentes foram ganho de peso gestacional excessivo, índice de massa corporal pré-gestacional ≥25 kg/m2 e diabetes mellitus gestacional. Conclusão: Os principais preditores para o excesso de peso ao nascer no Brasil são fatores de risco modificáveis. O estabelecimento de um estado nutricional adequado no período gestacional e mesmo após o parto parece ser a qualidade e a frequência do acompanhamento dos órgãos de saúde junto às mães e seus filhos.

Resumo O objetivo deste artigo é avaliar a condição nutricional de crianças com baixo peso ao nascer (BPN) e possíveis associações com variáveis independentes maternas, sexo e antecedentes neonatais Estudo transversal com 544 escolares com BPN (5 a 10 anos de idade) da região metropolitana de São Paulo. Variáveis: dados neonatais das declarações de nascidos vivos (peso ao nascer e idade gestacional), informações sobre a gestação e a condição nutricional atual das mães. A avaliação da condição nutricional dos escolares foi realizada por meio da obtenção dos dados de peso e estatura utilizados cálculo do escore z da estatura/idade (ZEI) e índice de massa corporal (ZIMC). Observou-se baixa estatura; sobrepeso e obesidade em 6,2%, 8,6% e 12,3% das crianças avaliadas, respectivamente. A presença de baixa estatura nos escolares associou-se com estatura materna < 150 cm (OR = 6,94; IC95% 2,34-20,6). O sobrepeso/obesidade nas crianças com BPN associou-se de forma independente com o sobrepeso/obesidade da mãe (OR = 2,40; IC95% 1,44-4,01) e o sexo masculino (OR = 1,77; IC95% 1,06-2,95). Um quinto dos escolares com BPN apresentaram excesso de peso, que se associou à condição nutricional materna atual e ao gênero masculino; a baixa estatura associou-se à estatura materna.


Abstract The scope of this study is to assess the nutritional status of low birth weight (LBW) children and the possible associations with independent maternal variables, gender and neonatal history. It involved a cross-sectional study with 544 LBW schoolchildren (five to ten years of age) in the metropolitan area of São Paulo. Variables: the neonatal data of liveborn infant declarations and the current weight and height of the mothers were collected. The weight and stature used to calculate the height/age z (HAZ) score and the body mass index (BMI) of children were evaluated. Among the LBW children 6.2% were of short stature, 12.3% overweight and 8.6% obese. There was an association between short stature in LBW schoolchildren and short maternal stature < 150 cm (OR = 6.94; 95 % CI 2.34-20.6). Excess weight/obesity in LBW children was independently associated with overweight/obesity of the mother (OR = 2.40; 95% CI 1.44-4.01), and the male gender (OR = 1.77; 95% CI 1.06-2.95). A fifth of schoolchildren with low birth weight were overweight, which was associated with current maternal nutritional status and the male gender and stunting was associated with maternal stature.

Abstract Objective To assess the association between dietary glycemic index (GI) and excess weight in pregnant women in the first trimester of pregnancy. Methods A cross-sectional study in a sample of 217 pregnant women was conducted at the maternal-fetal outpatient clinic of the Hospital Geral de Fortaleza, Fortaleza, state of Ceará, Brazil, for routine ultrasound examinations in the period between 11 and 13 weeks + 6 days of gestation.Weight and height were measured and the gestational body mass index (BMI) was calculated. The women were questioned about their usual body weight prior to the gestation, considering the prepregnancy weight. The dietary GI and the glycemic load (GL) of their diets were calculated and split into tertiles. Analysis of variance (ANOVA) or Kruskal-Walls and chi-squared (χ2) statistical tests were employed. A crude logistic regression model and a model adjusted for confounding variables known to influence biological outcomes were constructed. A p-value < 0.05 was considered significant for all tests employed. Results The sample group presented a high percentage of prepregnancy and gestational overweight (39.7% and 40.1%, respectively). InthetertilewiththehigherGIvalue, therewasa lower dietary intake of total fibers (p = 0.005) and of soluble fibers (p = 0.008). In the third tertile, the dietary GI was associated with overweight in pregnant women in the first trimester of gestation, both in the crude model and in the model adjusted for age, total energy intake, and saturated fatty acids. However, this association was not observed in relation to the GL. Conclusion A high dietary GI was associated with excess weight in women in the first trimester of pregnancy.


Resumo Objetivo Avaliar a associação entre índice glicêmico (IG) dietético e presença de excesso de peso em gestantes no primeiro trimestre de gestação. Métodos Estudo transversal realizado com 217 gestantes atendidas no Ambulatório de Medicina Materno-Fetal do Hospital Geral de Fortaleza, Fortaleza, CE, para realização de exames ultrassonográficos de rotina no período entre 11 e 13 semanas e 6 dias de gestação. Peso e altura foram obtidos para o cálculo do índice de massa corporal (IMC) gestacional. As mulheres foram questionadas quanto ao peso corporal habitual anterior à gestação, considerado o peso pré-gestacional. O IG e a carga glicêmica (CG) das suas dietas foram calculados e divididos em tercis. As associações foram investigadas por análise de variância (ANOVA, na sigla em inglês) ou pelos testes Kruskal-Walls e qui-quadrado (χ2). Resultados O grupo tinha alto percentual de excesso de peso pré-gestacional (39,7%) e gestacional (40,1%). Houve menor consumo de fibras totais (p = 0,005) e fibras insolúveis (p = 0,008) no tercil de maior valor de IG. No terceiro tercil, o IG da dieta foi associado ao excesso de peso dasmulheres no primeiro trimestre de gestação, tanto no modelo bruto como no modelo ajustado para idade, consumo total de energia e de ácidos graxos saturados. No entanto, não se observou esta associação emrelação à CG. Conclusão O alto IG da dieta consumida foi associado ao excesso de peso das mulheres no primeiro trimestre da gestação.

Introducción: El abandono precoz de la lactancia materna exclusiva se asocia con el desarrollo prematuro de exceso de peso y adiposidad. Objetivo: Determinar la posible relación entre el exceso de peso y la obesidad central con la duración de la lactancia materna exclusiva. Métodos: Estudio descriptivo que incluyó 162 adolescentes, entre 10-18 años, atendidos en el Policlínico Universitario Carlos Manuel Portuondo, durante el 2016. De las historias clínicas se tomaron los datos de las siguientes variables: edad, sexo, duración de la lactancia materna exclusiva, estado nutricional, grado de obesidad y adiposidad. Resultados: La malnutrición por exceso y por defecto fueron significativamente mayor en pacientes con cinco o menos meses de lactancia materna exclusiva (51,84 por ciento vs 9,26 por ciento respectivamente). El grado de obesidad y la adiposidad se relacionaron con el tiempo de duración de la lactancia materna exclusiva (p= 0,0131 y p= 0,0010 respectivamente). Del total de pacientes con adiposidad, el 81,39 por ciento presentó abandono de la lactancia materna exclusiva antes de los cinco meses de edad. Conclusiones: La lactancia materna exclusiva protege al niño (adolescente) frente al exceso de peso y la adiposidad a corto y mediano plazo(AU)


Introduction: Early dropping out of exclusive maternal breastfeeding it is associated with the premature development of excess weight and adiposity. Objective: To determine the possible relation among excess weight and central obesity with the duration of exclusive maternal breastfeeding. Methods: Descriptive study that included 162 adolescents from 10 to 18 years who were attended in Carlos Manuel Portuondo University Policlinic during 2016. From clinic records were taken the data of the following variables: age, sex, duration of exclusive maternal breastfeeding, nutritional state, stage of obesity and adiposity. Results: Malnutrition by lack or excess was significantly higher in patients with 5 or less months of exclusive maternal breastfeeding (51.84 percent vs. 9.26 percent respectively). Stages of obesity and adiposity were related with the duration of exclusive maternal breastfeeding (p= 0.0131 and p= 0.0010 respectively). Of the total of patients with adiposity, 81.39 percent presented dropping out of exclusive maternal breastfeeding before the five months of life. Conclusions: Exclusive maternal breastfeeding with a proper duration protects children (and then adolescents) of excess weight and adiposity(AU)

ABSTRACT

INTRODUCTION@#Laparoscopic sleeve gastrectomy (LSG) outcomes among adolescents and factors associated with adolescent obesity in Singapore were evaluated.@*METHODS@#Prospectively collected data of patients aged 16-19 years who underwent LSG was retrospectively reviewed. A lifestyle questionnaire, Berlin and Epworth scores, and Patient Health Questionnaire-9 scores were collected. Preoperative anthropometrics, comorbidities, weight loss and body composition outcomes were recorded.@*RESULTS@#Among 208 LSGs, 13 (6.3%) were performed on obese adolescents. Mean age and body mass index (BMI) at first presentation were 19.1 ± 0.9 (range 16.8-19.8) years and 46.2 ± 6.3 (range 36-57) kg/m, respectively. There was family history of obesity (n = 7) and regular consumption of high-calorie drinks (n = 12). Most patients had comorbidities (n = 12), including hypertension (n = 5), asthma (n = 4), diabetes mellitus (n = 3), hernia (n = 3) and obstructive sleep apnoea requiring continuous positive airway pressure support (n = 3). At one year, excess weight loss was 64.3% ± 34.7% (range 21.8%-101.5%), while BMI and fat mass dropped to 31.2 ± 7.6 (range 23-40) kg/m and 17.4 kg, respectively. Pain score was 2/10 at 24 hours after surgery. Mean postoperative stay was 2.7 days. No complications or readmissions occurred. Remission of diabetes mellitus and hypertension was reported in two of three and four of five adolescents, respectively, within one year of surgery.@*CONCLUSION@#LSG is a safe option for adolescents with good short-term weight loss outcomes and remission of metabolic comorbid conditions.

Abstract Objective: Investigate associations between excess weight in preschool children, breastfeeding duration and age of non-breast milk introduction. Methods: Cross-sectional study of a representative sample of 817 preschool children, aged 2-4 years, attending municipal day care centers in the city of Taubaté. The weight and height of children were measured in the day care centers in 2009, 2010 and 2011. The body mass index z-score (BMIz) was calculated and children were classified as risk of overweight (BMIz≥1 to<2) or excess weight (BMIz≥2). Data analysis was carried out by comparison of proportions, coefficient of correlation and multivariate linear regression. Results: The prevalence of risk of overweight was 18.9% and of excess weight (overweight or obesity) was 9.3%. The median duration of breastfeeding and age of introduction of non-breast milk was 6 months. The child's BMIz showed direct correlation with birth weight (r=0.154; p<0.001) and maternal body mass index (BMI) (r=0.113; p=0.002). The correlation was inverse with the total duration of breastfeeding (r=−0.099; p=0.006) and age at non-breast milk introduction (r=−0.112; p=0.002). There was no correlation between the child's BMIz with birth length, duration of exclusive breastfeeding and mother's age. Conclusions: The earlier the introduction of non-breast milk, the higher the correlation with excess weight at preschool age.


Resumo Objetivo: Investigar relações existentes entre excesso de peso em pré-escolares, duração do aleitamento materno e a idade de introdução de leite não materno. Métodos: Estudo transversal de amostra representativa de 817 pré-escolares, 2-4 anos de idade, de creches municipais de Taubaté. O peso e a altura das crianças foram mensurados nas creches em 2009, 2010 e 2011. Calculado o escore z de Índice de Massa Corporal (zIMC), as crianças foram classificadas como risco de sobrepeso (zIMC≥1 a<2) ou como excesso de peso (zIMC≥2). A análise dos dados foi feita por comparação de proporções, coeficiente de correlação e regressão linear multivariada. Resultados: A prevalência de risco de sobrepeso foi 18,9% e de excesso de peso (sobrepeso ou obesidade) de 9,3%. A mediana de duração do aleitamento materno e a idade de introdução do leite não materno foi de 6 meses. O zIMC da criança evidenciou correlação direta com o peso ao nascer (r=0,154; p<0,001) e com o Índice de Massa Corporal (IMC) materno (r=0,113; p=0,002). A correlação foi inversa com a duração total do aleitamento materno (r=−0,099; p=0,006) e a idade de introdução de leite não materno (r=−0,112; p=0,002). Não houve correlação entre o zIMC da criança com o comprimento ao nascer, duração do aleitamento exclusivo e idade da mãe. Conclusões: Quanto mais precoce a introdução de leite não materno, maior a correlação com excesso de peso na idade pré-escolar.

El desarrollo de obesidad y sobrepeso están influenciados por diversos factores. Se evalúa la relación entre exceso ponderal y algunas variables socio-económicas en trabajadores del sector salud. Se realizó un estudio diagnóstico, retrospectivo con una muestra no probabilística de 337 trabajadores de un centro de salud quienes fueron atendidos en una consulta de medicina preventiva. El promedio de edad fue 34,6 años, peso promedio 69,11 Kilogramos (Kg), estatura promedio 1,6 metros (m), Índice de Masa Corporal (IMC) promedio 26,66 Kg/m2. Treinta y seis por ciento de los trabajadores presentaban un IMC normal, 40,06% sobrepeso y 23,44% obesidad. No se encontraron diferencias significativas de peso entre géneros. El IMC promedio más alto lo tuvo el grupo entre 35-40 años (28,88 Kg/m2). El IMC promedio más alto lo tuvieron los trabajadores del horario nocturno (27,84 Kg/m2) y el más bajo los de fines de semana (24,14 Kg/m2). La mayor frecuencia de sobrepeso la presentaron los trabajadores matutinos (53,41%), y de obesidad los del nocturno (32,81%). El grupo de obreros no especializados tuvo el IMC promedio más alto (31,36 kg/m2); el grupo de gerentes tuvo un IMC promedio menor (26,73 Kg/m2). El exceso de peso afecta dos terceras partes de la muestra estudiada. Existe relación entre algunos factores socio-económicos y la frecuencia de exceso ponderal en el grupo estudiado(AU)


The development of obesity and overweight is influenced by several factors. The objective of this study was to assess the relationship between the frequency of overweight and obesity and various socioeconomic variables among health care workers from Caracas, Venezuela. A descriptive, retrospective study was conducted using a nonrandom sample of 337 workers seenat a local health center for preventive care. The mean age of the workers was 34.6 years, their mean weight was 69.11 Kg, their mean height was 1.6 m, and their mean body mass index (BMI) was26.66 Kg/m2. Among them, 36% had a normal BMI, 40.06% were overweight and 23.44% were obese. No significant difference in BMI was found between men and women. The group of 35 to 40-year-olds had the highest mean BMI (28.88 Kg/m2). The highest mean BMI was found among those who worked the night shift (27.84 Kg/m2), and the lowest mean BMI among those who worked the weekend shift (24.14 Kg/m2). The highest frequency of overweight was found among those who worked the morning shift (53,41%), and the highest frequency of obesity among those working the night shift (32.81%). Unskilled workers had a mean BMI of 31.36 kg/m2, and those who worked in the most senior positions had a mean BMI of 26.73 kg/m2. Overall, excess weight affected two-thirds of the study sample. There is a relation ship between socioeconomic factors and frequency of excess weight in the study population(AU)

Introduction Insoles are used for different purposes and they can be an additional factor aiding weight loss in obese people due to their contribution in reducing musculoskeletal pain. Objective The aim of the present study was to assess the effect of custom insoles for redistributing plantar pressure, decreasing musculoskeletal pain and reducing postural changes in obese adults. Materials and methods Data was obtained from morbidly obese people, who had a Body Mass Index (BMI) ≥ 35 and < 45 kg/m2, at three moments: before the insole was provided, immediately after insole use began and after two months of use. Anthropometric, postural, musculoskeletal pain and computerized baropodometry data were collected. Results When comparing the stages before the use of the insole and after two months of use, significant (p < 0.05) decreases were observed in the number of people with right lateral postural misalignment, in the intensity of the musculoskeletal pain of the body segments evaluated (except for the right knee), and in the mean of the peak plantar pressure values. In terms of total plantar contact area, the mean increased significantly (p < 0.05) in both right and left feet after the intervention. Conclusion This study demonstrated that the daily use of proprioceptive insoles contributed to reducing peak plantar pressure, musculoskeletal pain and lateral postural deviations. The use of proprioceptive insoles may be an important strategy to encourage obese people to exercise and, consequently, reduce weight. .


Introdução Palmilhas têm sido utilizadas por diferentes razões, e podem auxiliar na perda de peso em pessoas obesas por contribuírem na redução da dor musculoesquelética. Objetivos O objetivo do presente estudo foi avaliar o efeito da palmilha na redistribuição da pressão plantar, na diminuição da dor musculoesquelética e na redução das alterações posturais em adultos obesos. Materiais e métodos Os dados foram obtidos de pacientes com obesidade mórbida e Índice de Massa Corporal (IMC) ≥ 35 e < 45 kg/m2, em três diferentes momentos: antes do fornecimento da palmilha, imediatamente após o uso da palmilha e após dois meses de uso da palmilha. Foram coletados dados relacionados à antropometria, postura, dor musculoesquelética e baropodometria computadorizada. Resultados Quando comparadas as etapas antes do uso das palmilhas e após dois meses de uso, houve redução significativa (p < 0,05) no número de indivíduos com desvio lateral direito, na intensidade da dor musculoesquelética dos segmentos corporais avaliados (exceto para o joelho direito) e nos valores médios dos picos de pressão plantar. No que tange à área total de contato plantar, sua média aumentou de forma significativa (p < 0,05) em ambos os pés, direito e esquerdo, após a intervenção. Conclusão Este estudo demonstrou que o uso diário de palmilhas proprioceptivas contribuiu na redução dos picos de pressão plantar, da dor musculoesquelética e dos desvios posturais laterais. O uso de palmilhas proprioceptivas pode ser uma importante estratégia para estimular indivíduos obesos a praticarem exercícios e, consequentemente, reduzirem seu peso. .

Introduction: At one year of age, factors that favor the development of obesity and other metabolic disorders in later stages of life are present. Objective: To identify perinatal, societal, and dietary risk factors for excess weight and obesity in 12 months infants. Subjects and methods: Case-control studies with infants under the care of Family Medicine Units. These cases studied were overweight or obese children, and control cases were children at a healthy weight. Each group included 152 infants, who were evaluated based on their responses to a questionnaire, their records, and perinatal, societal, and dietary variables. Results: The variables with statistical significance were high pre-gestational weight, high gestational weight gain (OR 1.79-3.88), absence of breastfeeding and exclusive formula feeding (OR 2.02 and 1.75). Conclusion: In the presence of pre-pregnancy obesity and exclusive formula feeding, there is a 64.4% greater risk of becoming overweight or obese at 12 months old.


Introducción: Al año de vida existen factores que favorecen el desarrollo de obesidad y alteraciones metabólicas en etapas posteriores. Objetivo: Identificar factores de riesgo perinatales, sociales y alimentarios de sobrepeso y obesidad en lactantes de 12 meses de edad. Sujetos y método: Estudio de casos y controles en lactantes, que acuden a Unidades de Medicina Familiar, considerando casos; niños con sobrepeso u obesidad y controles: peso adecuado. Se incluyeron 152 lactantes por grupo, a los cuales se les estudió mediante un cuestionario y revisión de expedientes, variables perinatales, sociales y alimentarias. Resultados: Las variables con significancia estadística fueron: peso pre-gestacional de la madre elevado, incremento de peso gestacional elevado; OR 1,79 y 3,88 ausencia de lactancia materna y alimentación con fórmula láctea exclusiva, OR 2,02 y 1,75. Conclusión: en presencia de obesidad pregestacional y alimentación con formula láctea exclusiva, existe un riesgo 64,38% mayor de desarrollar sobrepeso u obesidad a los 12 meses de edad.

OBJECTIVES: To investigate the prevalence of excess body weight in the pediatric ward of University Hospital and to test both the association between initial nutritional diagnosis and the length of stay and the in-hospital variation in nutritional status. METHODS: Retrospective cohort study based on information entered in clinical records from University Hospital. The data were collected from a convenience sample of 91 cases among children aged one to 10 years admitted to the hospital in 2009. The data that characterize the sample are presented in a descriptive manner. Additionally, we performed a multivariate linear regression analysis adjusted for age and gender. RESULTS: Nutritional classification at baseline showed that 87.8% of the children had a normal weight and that 8.9% had excess weight. The linear regression models showed that the average weight loss z-score of the children with excess weight compared with the group with normal weight was −0.48 (p = 0.018) and that their length of stay was 2.37 days longer on average compared with that of the normal-weight group (p = 0.047). CONCLUSIONS: The length of stay and loss of weight at the hospital may be greater among children with excess weight than among children with normal weight. .

Objective: to evaluate the relationship between body composition of preschool children suffering from excess weight and birth weight (BW). Methods: probabilistic sample, by conglomerates, with 17 daycare centers (of a total of 59) composing a final sample of 479 children. We used Z-score of Body Mass Index (zBMI) ≥ +1 and ≥ +2, respectively, to identify preschool children with risk of overweight and excess weight (overweight or obesity). The arm muscle area (AMA) and the arm fat area (AFA) were estimated from measurements of arm circumference, triceps skin fold thickness. Results: the prevalence of risk of overweight was 22.9% (n=110) and excess weight was 9.3% (n=44). The risk of overweight and excess weight in children did not show correlation between BW and AFA, but it did with adjusted arm muscle area (AMAa) (rp= 0.21; p= 0.0107). The analysis of the group with excess weight alone also showed a positive correlation between BW and AMAa (rp= 0.42; p= 0.0047). Conclusion: among overweight children, lower BW is associated with a lower arm muscle area in early preschool age, regardless of the fat arm area presented by them. .


Objetivo: avaliar a relação entre a composição corporal de pré-escolares portadores de excesso de peso e o seu peso ao nascimento (PN). Métodos: amostra probabilística, por conglomerados, com 17 creches (de um total de 59), composta por 479 crianças. Para identificar os pré-escolares portadores de risco de sobrepeso e de excesso de peso (sobrepeso ou obesidade), utilizou-se o escore Z de índice de massa corpórea (zIMC) ≥ +1 e ≥ +2, respectivamente. A área muscular do braço corrigida (AMB) e a área gorda do braço (AGB) foram estimadas a partir das medidas de circunferência do braço e da dobra cutânea tricipital. Resultados: a prevalência de risco de sobrepeso foi de 22.9% (n= 110) e de excesso de peso de 9.3% (n= 44). Juntas, as crianças com risco de sobrepeso e excesso de peso não mostraram correlação entre o PN e a área gorda do braço, mas, sim, com a área muscular do braço corrigida (AMBc) (rp= 0.21; p= 0.0107). A análise só do grupo com excesso de peso também evidenciou correlação positiva entre PN e AMBc (rp= 0.42; p= 0.0047). Conclusão: nas crianças com excesso de peso, um menor PN está associado a uma menor área muscular do braço no início da idade pré-escolar, independentemente da área gorda do braço que elas apresentem. .

The aim of this study was to investigate the correlation between physical activity measured by accelerometry and excess weight in schoolchildren. Three hundred and ninety one school-age adolescents (10 to 18 years old) participated in the study. The cut-off points used to estimate time spent in physical activity were: moderate ≥3.0 METs and vigorous ≥6.0 METs. Student's t-test and Pearson product-moment correlation coefficient were used to verify statistical differences and correlations between physical activity and body mass index (BMI). Statistical significance was set at p<0.05. Male schoolchildren spent more time in moderate (96.1 ± 39.6 min.day-1) and vigorous (9.7 ± 8.8 min.day-1) physical activity than their female peers (moderate: 73.7 ± 37.7 min.day-1; vigorous: 6.1 ± 6.8 min.day-1). For both sexes, younger schoolchildren (10 to 14 years old) were more physically active than older ones (14 to 18 years old). Time spent in moderate-vigorous physical activity was inversely related to BMI. These findings suggest that regular physical activity (RPA) is related to body weight reduction in schoolchildren. Therefore, RPA can be used as an obesity prevention strategy in elementary school.


O objetivo do presente estudo foi investigar a associação da atividade física medida pelo acelerômetro com o excesso de peso corporal em escolares. Trezentos e noventa e um adolescentes em idade escolar (10 a 18 anos) participaram deste estudo. Os pontos de corte utilizados para determinar o tempo gasto em atividade física foram: moderada ≥ 3,0 METs e vigorosa ≥ 6,0 METs. Teste t de Student e coeficiente de correlação produto-momento de Pearson foram utilizados para verificar diferenças estatísticas e associações entre atividade física e IMC. A significância estatística foi fixada em p <0,05. Escolares do sexo masculino passaram mais tempo em atividade física moderada (96,1 ± 39,6 min/dia) e vigorosa (9,7 ± 8,8 min/dia) do que o sexo feminino (moderado: 73,7 ± 37,7 min/dia; vigorosa: 6,1 ± 6,8 min/dia). Para ambos os sexos, os escolares mais jovens (10 a <14 anos) foram mais ativos fisicamente do que os escolares mais velhos (14 a <18 anos). Tempo despendido em atividade física moderada e vigorosa foi inversamente relacionado ao IMC. Estes achados sugerem que a prática regular de atividade física está relacionada a redução do peso corporal em escolares. Por conseguinte, pode ser usado como uma estratégia de prevenção da obesidade na escola elementar.

OBJETIVO: Estudar a prevalência e os fatores associados ao excesso de peso em crianças matriculadas em escolas públicas dos estados do Rio Grande do Sul (RS) e Santa Catarina (SC). MÉTODOS: Realizou-se estudo transversal com crianças de idade entre quatro e seis anos. O desfecho estudado foi o excesso de peso, definido através do escore Z > 2DP para o Índice de Massa Corporal (IMC)/idade, em comparação com a população de referência da OMS 2006/2007. As medidas antropométricas de massa corporal e altura foram aferidas em duplicata, utilizando-se técnicas padronizadas conforme a Organização Mundial de Saúde (OMS). Os dados foram duplamente digitados utilizando o software EPI-INFO, versão 6.04. Foram calculadas frequências absolutas e relativas e médias (DP). Associações entre excesso de peso e demais variáveis foram avaliadas em modelo de Poisson de variância robusta. Foi utilizado o programa STATA versão 12.0 (p < 0,05). RESULTADOS: Foram avaliadas 4.914 crianças (RS 2.578 e SC 2.336). No RS, o excesso de peso foi de 14,4% (IC 95% = 13,1-15,8%) e, em SC, de 7,5% (IC 95% = 6,5-8,7%). As variáveis que apresentaram associação com o excesso de peso foram: número de moradores no domicílio; escolaridade materna; situação conjugal; número de filhos; idade materna ao nascimento do primeiro filho; idade gestacional; e o peso ao nascer. CONCLUSÃO: As crianças matriculadas nas pré-escolas públicas do RS apresentaram uma prevalência de excesso de peso duas vezes maior do que a identificada em SC, demonstrando uma diferença significativa na magnitude da obesidade infantil em dois estados brasileiros situados em uma mesma região.


OBJECTIVE: To study the prevalence and factors associated with excess weight in children enrolled in public schools in the states of Rio Grande do Sul (RS) and Santa Catarina (SC). METHODS: This was a cross-sectional study, carried out with children aged 4 to 6 years. The studied outcome was excess weight, defined by z-score > two standard deviations for body mass index (BMI)/age, compared with the World Health Organization (WHO) reference population of 2006/2007. Anthropometric measurements of body mass and height were measured in duplicate using standard techniques, in accordance with the WHO. Data were double entered using EPI-INFO software, release 6.04. Absolute and relative frequencies were calculated, as well as mean values and standard deviations. Associations between excess weight and other variables were assessed by using Poisson model with robust variance. STATA software release 12.0 was used (p < 0.05). RESULTS: A total of 4,914 children were evaluated (2,578 in RS and 2,336 in SC). In RS, the incidence of excess weight was 14.4% (95% CI = 13.1% to 15.8%) and in SC, 7.5% (95% CI = 6.5% to 8.7%). The variables associated with excess weight were number of household members, maternal education, marital status, number of children, mother's age at birth of first child, gestational age, and birth weight. CONCLUSIONS: Children enrolled in public preschools in RS had a two-fold higher excess weight prevalence than that identified in SC, demonstrating a significant difference in the magnitude of childhood obesity in two Brazilian states located in the same region.

ABSTRACT

OBJETIVO: Avaliar a validade do peso, altura e índice de massa corporal (IMC) autorreferidos em adultos de Brasília e identificar os fatores associados às diferenças entre os valores medidos e autorreferidos. MÉTODOS: Em amostra por conglomerados de 250 domicílios foram aferidos peso e altura de todos os adultos residentes, após entrevista. A concordância entre os parâmetros medidos e autorreferidos foi feita por Bland & Altman (B&A). Utilizou-se análise de variância múltipla e estimou-se a sensibilidade e especificidade para o excesso de peso (IMC ? 25 kg/m2), estratificado por anos de estudo e faixa etária. RESULTADOS: As entrevistas e mensurações foram feitas em 469 indivíduos. Os resultados de B&A mostram viés (diferença dos valores medidos e autorreferidos) não significativo para o peso (-0,17 kg, p = 0,1) e significativo para a altura (-1 cm, p < 0,001). Os limites de concordância (LC) foram amplos: de ±4 kg e de -6 cm a 4 cm. O viés do IMC foi de - 0,06 kg/m2 e não significativo (p = 0,08) e os LC de - 1,5 a +1,4 kg/m2. Observou-se que homens com IMC < 25 kg/m2 superestimam o peso, enquanto aqueles com sobrepeso e obesos o subestimam (p < 0,01). Homens com + 60a de idade (p = 0,037) e mulheres com < 12a de estudo (p < 0,01) superestimam a altura. A sensibilidade e a especificidade para o excesso de peso foram para homens e mulheres, respectivamente: 94% e 88%; 90% e 98%. A sensibilidade foi de 77% para mulheres acima de 60 anos, de 75% para mulheres com 9 a 11 anos de estudo, e a especificidade de 78% para homens entre 30 e 39 anos. CONCLUSÃO: Em Brasília, a altura autorreferida pode ser utilizada para subgrupos de homens < 60anos de idade e mulheres com + 12 anos de estudo. Homens com IMC adequado ...


OBJECTIVE: To assess the validity of self-reported weight, height and body mass index (BMI) estimates from adults in Brasilia and to identify factors associated to differences between measured and self-reported values. METHODS: In a cluster sampling 250 household´s were selected and weight and height were measured in all adults after an interview. Bland & Altman (B&A) was used to determined agreement between measured and self-reported parameters. The multiple analysis of variance was used to determine the associated factors. Sensitivity and specificity were used for excess weight (BMI ≥ 25 kg/m2). RESULTS: Interviews and measurements were done in 469 individuals. B&A showed bias (difference between measured and self-reported values) was not significant for weight (- 0.17 kg, p = 0.1) and significant for height (- 1 cm, p < 0.001). The limits of agreement (LA) were wide: from ± 4 kg for weight and - 6 cm to 4 cm for height. BMI presented a non significant bias of - 0.06 kg/m2 (p = 0.08) and LA of - 1.5 to + 1.4 kg/m2. Men with a BMI < 25 kg/m2 overestimated weight, while overweight or obese men underestimated it (p < 0.01). Men older than 60y of age (p = 0.037) and women with less than 12y of study (p < 0.01) overestimated height. The sensitivity and specificity to detect excess weight were for men and women, respectively: 94%, 88% and 90%, 98%. Sensitivity to detect excess weight was 77% for 60 or over years old women and 75% for women between 9 to 11 years of study, while. specificity was 78% for men between 30 e 39 years old. CONCLUSION: In Brasilia, self-reported height can be used for subgroups of women with more than 12y of study and men under 60y of age. Men with adequate BMI over estimate their weight when compared to overweight and obese men. Self-reported measures can be used for excess weight population follow-up. .
